What Is CIRS?

Chronic Inflammatory Response Syndrome is a complex, multi-system illness that affects millions of people — yet remains widely misunderstood and misdiagnosed. Understanding CIRS is the first step toward reclaiming your health.

Understanding CIRS

Chronic Inflammatory Response Syndrome (CIRS) is an acquired illness triggered by exposure to biotoxins — naturally occurring toxins produced by organisms such as mold, bacteria, and certain marine life. It is not an allergy or a psychological condition; it is a measurable, physiological illness driven by a dysregulated immune response.

CIRS was identified and characterized by Dr. Ritchie Shoemaker, a pioneer in biotoxin-related illness research. His work established that approximately 25% of the population carries specific HLA-DR gene variants that prevent the body from recognizing and clearing biotoxins. In these individuals, biotoxins recirculate indefinitely, triggering a cascade of chronic inflammation that affects virtually every body system.

Because CIRS mimics dozens of other conditions — chronic fatigue syndrome, fibromyalgia, Lyme disease, anxiety disorders, and more — patients often spend years seeking answers before receiving an accurate diagnosis. Many are told their symptoms are "all in their head." They are not.

Common Symptoms

CIRS is a multi-system illness, meaning it can affect nearly every part of the body. Symptoms vary widely from person to person and often fluctuate in severity. Common presentations include:

  • Persistent fatigue and weakness
  • Cognitive impairment ("brain fog"), memory issues, and difficulty concentrating
  • Headaches and light sensitivity
  • Muscle aches, joint pain, and morning stiffness
  • Shortness of breath and chronic sinus congestion
  • Mood changes, anxiety, and depression
  • Numbness or tingling sensations
  • Unusual thirst and frequent urination
  • Temperature dysregulation
  • Digestive issues and abdominal pain

A note on skin symptoms:Skin symptoms can sometimes be one part of a larger picture. While many skin conditions have common dermatologic causes, some individuals with CIRS also report persistent or unexplained skin symptoms alongside fatigue, brain fog, sinus issues, and other systemic concerns. If you're experiencing multiple ongoing symptoms that don't seem to fit together, it may be worth discussing environmental exposures with your healthcare provider.

Note: This list is not exhaustive. CIRS can present with over 30 documented symptom clusters. If you recognize yourself in many of these symptoms, a formal evaluation may be warranted.

What Triggers CIRS?

CIRS is initiated by exposure to a biotoxin. The most common trigger by far is a water-damaged building, but several other sources have been identified:

Water-damaged buildings

Mold and mycotoxins from buildings with past or present water damage are the most common trigger.

Lyme disease

Borrelia and co-infections can trigger a biotoxin response in genetically susceptible individuals.

Ciguatera fish poisoning

A toxin found in certain tropical fish that can initiate the CIRS cascade.

Pfiesteria

A dinoflagellate found in coastal waters linked to CIRS in some patients.

Brown recluse spider venom

Venom from this spider can act as a biotoxin trigger in susceptible individuals.

How CIRS Develops — and How It Is Treated

Understanding the mechanism behind CIRS helps explain why it affects so many body systems — and why a systematic, step-by-step treatment approach is necessary.

Genetic susceptibility

About 25% of the population carries HLA-DR gene variants that prevent the immune system from properly clearing biotoxins, leaving them to recirculate and cause ongoing inflammation.

Biotoxin exposure

Exposure to a biotoxin source — most commonly a water-damaged building — introduces toxins into the body. Studies estimate that up to 50% of homes and workplaces show evidence of past or current water damage, making this a far more common exposure than most people realize.

Innate immune activation

The innate immune system mounts a response, releasing inflammatory cytokines. In susceptible individuals, this response does not shut off normally.

Chronic inflammation

Persistent cytokine elevation affects multiple body systems — the brain, hormones, immune regulation, and more — producing the wide range of CIRS symptoms.
